Asteroids Deluxe Monitor Overlay

Anyone want to send me some pictures of their original Asteroids Deluxe
monitor film overlay? I'm looking at making a limited run of them, but
don't have an original as a template.

Any pics, dimensions, colors would be greatly appreciated
