Asteroids rev 5 board - component values needed please

As the AD561J DAC on one of my Asteroids boards has failed I decided to
replace it with a DAC312HP in the vacant location as provided on later revs
of Asteroids boards.

However, I now notice that this won't work as some of the leads go to some
vacant locations for resistors and capacitors.

Would someone with rev 5 (or later) Asteroids schematics and/or a later
manual please tell me the type, value, etc of components at the following
locations:

R147
R148
R149
R150
R151
R152
C124
C125
C126
C127
C128

Also, if the schematics/notes have any text referring to the use of an
AM3012 (that's what the DAC312HP replaces) then please can you let me know
what they are.
