15" tube replacement.

I have a 15" battlezone monitor (g05) with a dead tube,

the original sylvania part is not listed anywhere on the net.
(well - except on mark jenison's site :-) )

does anybody have the details of this tube?
gun voltages,grid voltages, HT voltage etch?

or does anybody know of any compatable tubes?

i dont mind changing the neck connector, as long as the yoke will fit.

also, if anybody knows where to get one / has one available in the u.k. ???
