You can already do this with a combination of entries in the vmmenu.ini file and getting creative with the vmm.bat file.
e.g. in vmmenu.ini
Other|ZVG Tweak|zvgtweak|zvgtweak
and in vmm.bat:
if %1 == zvgtweak GOTO zvgtweak
... other code here as usual
goto end
rem Run ZVGTweak if selected from menu
:zvgtweak
c:\zvg\zvgtweak.exe
goto end
...other code for other additions...
:end

I've updated the DOS ZVG Vector Mame Menu, adding some new features:
* spinner/mouse support
* customizable colours
* support for old versions of mame lacking the zvg_menu switch
* smarter navigation mode option
* keyboard led support
I forgot to announce the release of v1.1 a week or so back, this added, amongst other things:
* full screen rotation
* customizable controls
Finally you Tempest owners should be catered for :)
