I decided to forgo the HC4040 conversion for now. Too much risk given my desire to get a working prototype on the first pass.

The active components (switches, linear pot, adjustment pots, jumpers, and video connector) are all populated on the reverse side. This will allow me to put the board on standoffs and a acrylic top on the device to protect it from dust/flying multimeter/scope probes.
While the board is likely smaller than the two in the original; the board is still 5.1x3.9” tall which yields a board costing over $50 at batchPCB. By far the largest parts on the board is the 2716s... oneday maybe I’ll figure out how to move the EPROMS into a single EEPROM device and surface mount it.
I’m still moving forward with the project; will likely be ordering the prototype board tomorrow afternoon.

Yes should be U2 instead of U3.

No reason to not leave U1d and U6b as is. Just don't connect HC4040 pin 1 at all (U6 pin 9 will provide A8).
